Sapphire Hospice is committed to providing a continuum of care for your patients with life-limiting illnesses. We serve patients and families in the metro Atlanta, GA area with palliative care, skilled nursing, MD/NP home visits, medical social work for emotional support and case management, HHA to assist with personal care, therapies as directed by a hospice MD, and volunteers to assist as needed.

We will work closely with your team to ensure the highest quality care for your patient, and to find the right place for care. We tailor a care plan especially for each patient, with sensitivity to cultural, religious, and personal preferences. Our well-trained volunteers and dedicated staff, including physicians who make home visits, help ensure that our patients spend their final months in comfort and with the support they need.

We can also help your patients plan for their future care. We know these conversations can be hard – our team has the time, expertise, and tools to talk to your patients and help them complete their advance directives.

Patients are referred through hospitals, physicians, caregivers, and family members. Staff are also available for informational visits in hospitals, nursing homes, assisted living facilities, and private homes, for individuals and families considering hospice or palliative care.

Our expert educators and clinicians also provide continuing education for healthcare providers, including training for nursing and social work students, hospice rotations for medical students, and in-services on a range of subjects for staff at care homes and assisted living facilities.
